Athletic Registration for School Year 2025-2026
Be prepared by having all your athlete's paperwork into the office. These forms must be FULLY completed and signed and turned into the Office with payment, BEFORE the first day of practice. Hover over the ATHLETIC TAB for each Sports Information including Start Date, Schedules and Coaches. Note: Athletes should be registered for school by the first day of practice.
1. A current IHSA Sports Physical Form (exception 6th and 9th graders can use your School Physical)
2. Athletic Permission Form SY26 This form must have ALL information including insurance, and signatures
3. IHSA Sports Medicine Acknowledgment & Consent Form
4. Student Drug-Testing Procedures (Only 9-12th grade)
5. Sports Equipment Fees Paid to the office when turn in your Registration: (There are no Travel Fees)
High School: $100 per first two sports, $50 for third
Junior High: $75 per first two sports, $50 for third
Monroe Clinic Sports Medicine services our student athletes with certified Athletic Trainers during the every season.
SCHOOL SPORTS PARTICIPANTS NEED AN EVALUATION. Both Illinois and Wisconsin require children participating in school sports to have an evaluation prior to play time and it is recognized by all as a necessary step for participating.
