New Students to the district are asked to call the office (Elementary or High School) and make an appointment to meet with a school official and/or Guidance Counselor/Principal.

The 7-12 High School will be taking appointments on Wednesdays.

Bring these documents along with you to your appointment:
1. Proof of Residency
2. Birth Certificate
3. Child Custody Legal Verification (if applicable)
4. Transcripts (if available)

State Medial Forms and requirements:
PK- Physical and current immunizations required to enter preschool.
KG- Physical/ current immunizations, dental exam, and a comprehensive eye exam required to enter KG.
2nd - Dental exam
6th- Physical/current immunizations and dental exam required to enter 6th grade
9th- Physical/current immunizations and dental exam required to enter 9th grade
Please turn the State of Illinois physical forms filled out in their entirety, signed by the physician and parent into the school office as soon as possible. We do not wish to exclude any child from school but the exclusion date is October 15th. All forms should be turned in prior to that date.
